Current estimates indicate that chronic obstructive pulmonary disease (COPD) affects roughly two million Canadians, with an additional million individuals potentially living with the condition undiagnosed and untreated.
Data from the Canadian Institute for Health Information shows that COPD accounts for about 76,000 annual hospital admissions, a figure that trails only behind the number of births in the country.
The Two Main Forms
The primary manifestations of COPD are emphysema and chronic bronchitis, which frequently coexist.
Emphysema involves damage to the tiny air sacs—known as alveoli—impairing the lungs’ ability to transfer oxygen into the bloodstream.
Chronic bronchitis is characterized by inflammation of the airway lining, which narrows airflow and leads to excess mucus production.
The Human Side of COPD
Fear and Stigma
Many patients arrive at clinics driven by fear, worrying about oxygen depletion, suffering, symptom burden, and an increased risk of lung cancer.
Dr. Pierre Landry, a respirologist at Dartmouth General Hospital in Halifax, notes that the stigma attached to smoking can add shame, further complicating early diagnosis and treatment.
Clinical Challenges and Hope
Beyond the physical symptoms, COPD raises the likelihood of lung cancer and may necessitate a lung transplant.
Dr. Landry explains that some individuals recall a parent on oxygen, underscoring the distressing nature of the disease, and his role includes educating patients about the disease process and tailoring effective therapies.
He also emphasizes the importance of building long‑term relationships with patients, including end‑of‑life support.
Research and Treatment Gaps
While conditions such as heart disease and stroke attract substantial research funding, COPD has historically been “swept under the rug,” often dismissed as a consequence of long‑term smoking.
Consequently, the evolution of COPD treatments has been slower than that of cardiac medications, though new biologic drugs now offer symptom relief and renewed hope for patients.
